With the release of the DLC the bunker spawning has been changed. There are a total of 9 bunkers in 9 locations, 3 of which will be filled. Locations: Ash Canyon. Bleak Inlet. Forlorn Muskeg. Hushed River Valley. I just gave up on a save in a Coastal Highway start. I searched the entire map, all fishing shacks, bodies, buildings, the campsite, the abandoned lookout - no hatchet. Also no rifle. Since I started the game with no water, just come cold coffee, it was a struggle just to be able to search every location. That seems a little bit harsh for a Voyager start.
